Ana Quiñones\, PhD and David Dorr\, MD will be presenting on “Preliminary Findings from Person-Centered Care Planning for People with Multiple Chronic Conditions (PCCP4P).” This webinar will discuss findings from the PCCP4P project\, including a high-level overview of models and approaches to providing person-centered care planning (PCCP) for people with multiple chronic conditions (MCCs).
