GUIDE+ACP Video Intervention Implementation in Nursing Homes

Dr. Chisholm is an associate professor in the School of Global Health Management & Informatics at the University of Central Florida. Dr. Chisholm’s research interests include long-term services and supports, quality of care, disparities, dementia care, and implementation and dissemination. Her research focuses on mitigating racial and ethnic disparities in nursing homes, particularly among residents living with dementia. This Investigator Training Award to Promote Diversity will provide Dr. Chisholm with necessary training in pragmatic trial methodology and intervention development to test interventions for effective advance care planning for nursing home residents with dementia and their surrogate decision makers to ensure all residents have access to high-quality care.

Advance care planning (ACP) is associated with improved palliative care outcomes in nursing homes (NHs). However, ACP in NHs is inadequate, with inconsistent ACP discussions and limited documentation of residents' preferences. The overall goal of this study is to develop an ACP guide and evaluate the perceptions of NH staff on implementing the guide and an ACP video intervention. The Investigator Training Award to Promote Diversity will provide Dr. Chisholm with the necessary experience, training, and mentorship to develop expertise in embedded pragmatic clinical trials (ePCTs) by conducting a pilot study to collect preliminary data in preparation for a future trial.  The training will support the following Specific Aims: (1) To develop the GUIDE with ACP champions to improve adherence of the ACP video intervention with people living with dementia (PLWD) and surrogate decision makers (SDMs); (2) To evaluate NH staff's perceived use and implementation of the GUIDE+ACP video intervention; and (3) To describe the strengths and challenges of developing an ACP intervention pilot study for an ePCT among NH staff, PLWD, and SDMs that will guide the development of a future study. This award will advance Dr. Chisholm’s development as an independent investigator with expertise in aging and ePCTs to improve health outcomes for older adults, particularly those living with dementia.
