Learning Health Network

The IMPACT Alzheimer’s disease and Alzheimer’s disease-related dementias (AD/ADRD) Learning Health Network is a consortium of individuals representing care settings and organizations joined by the common goal of improving care quality, equity and accessibility for people living with dementia and their care partners. The Learning Health Network is overseen and supported by the IMPACT Collaboratory: A group of health systems leaders and researchers dedicated to bringing together diverse health organization stakeholders to improve the care of people living with dementia and their care partners.

The Learning Health Network is comprised of four communities:

  • Long-term Care (nursing homes, assisted living, and other settings that provide around-the-clock care for persons living with dementia)
  • Healthcare Settings (Clinics, ACOs, MA Plans, Integrated Delivery Systems)
  • Hospitals/Emergency Departments
  • Community-based Organizations (Meals on Wheels, Hospice, Senior Centers, Adult Day Centers)

Organizations in the Learning Health Network engage to share experiences and generate knowledge to improve care for people living with dementia and their care partners. Organizations that join the Learning Health Network benefit in multiple ways:

  • Participate in a setting specific community to explore shared questions and interests
  • Receive cutting-edge information about relevant practice changes to meet the needs of caring for people with dementia and their care partners
  • Access experts in dementia care and learn how to take your existing data and make it useful in a meaningful way
  • Have a voice in identifying the biggest challenges caring for people with dementia and their caregivers and be a part of finding potential solutions
  • Partner in non-pharmacological research projects embedded within your care setting to address a problem that you are facing in delivering dementia care
