Care Fragmentation, Care Continuity, and Care Coordination—How They Differ and Why It

IMPACT member, Julie Bynum, MD, MPH, co-authored a viewpoint article addressing health care fragmentation, or the practice of a single patient seeing many physicians for their health care needs. Despite common occurrences, there has been limited scientific scrutiny around fragmentation of care and its potential harms.  Authors encourage a national dialogue about health care delivery. They consider why fragmentation occurs and they suggest a potential path forward for developing evidenced-based strategies that can reduce the occurrence of fragmentation and its associated harms.
