IMPACT Pilot Grant Awardee, Brent Forester, MD, MSc, and team have published a second article resulting from the IMPACT-funded CRESCENT (CaReEcoSystem primary Care Embedded demeNtia Treatment study.  CRESCENT is a telephone-based dementia care intervention adapted from the Care Ecosystem model aimed at equipping nurse care managers (NCMs) with tools necessary to provide care for people living with dementia (PLWD) and to interact with family caregivers.

The recent publication, “A Pilot Trial of CRESCENT in a Large Academic Healthcare System: A CaReEcoSystem Primary Care Embedded DemeNtia Treatment,” demonstrates the feasibility of collecting emergency department visit data through Medicare claims and identifies challenges faced by NCMs in implementing the CRESCENT intervention.  The authors highlight the importance of value-based dementia care models in enhancing the quality and efficiency of dementia care for patients and caregivers while containing healthcare costs.

Forester’s previous publication, “Impact of dementia care training on nurse care managers’ interactions with family caregivers” introduced the CRESCENT intervention and reported on intervention fidelity and insights gained about how dementia care training impacted NCMs’ provision of dementia care management and interactions with caregivers of PLWD.
