Gaps in the coordination of care for people living with dementia

IMPACT Demonstration Project Awardee, Lisa Kern, MD, MPH, and team have published survey results of a cross-sectional study exploring care coordination for people living with dementia (PLWD).
The cross-sectional study, conducted within a New York Medicare Accountable Care Organization (ACO), examined perceptions of care coordination and adverse events among PLWD and their caregivers. The research team used an existing survey to assess perceived communication gaps and adverse events like repeat tests, drug interactions, emergency department visits, and hospital admissions among PLWD with fragmented care. Findings revealed that 57% perceived gaps in care coordination, and 18% reported adverse events attributed to these gaps. The study highlights the prevalence and potential hazards of poor care coordination for PLWD.
