Study shows that pharmacist-led deprescribing via telehealth can reduce burden on PLWD

Pharmacist-led telehealth deprescribing for people living with dementia and polypharmacy in primary care: A pilot study

The recent article in the Journal of the American Geriatrics Society (JAGS) by IMPACT funded investigator Ariel Green, MD, MPH, PhD and team, describes how complexities in medication management can be reduced and patient goals can be met when pharmacists are involved with medication administration for people living with dementia (PLWD).

Authors describe the pilot study of ALIGN: Aligning Medications with What Matters Most, to optimize medications with patient care partner priorities. The study ran from May 2021- 2022 in two health care systems. Researchers found that pharmacist-led deprescribing and coordination via telehealth can reduce the overall medication burden for PLWD and help meet patient-care goals.
