Program Overview

The National Institute on Aging (NIA) Imbedded Pragmatic Alzheimer’s Disease and Alzheimer’s Disease-Related Dementias (AD/ADRD) Clinical Trials (IMPACT) Collaboratory (U54AG063546) was established in 2019 to build the nation’s capacity to conduct embedded pragmatic clinical trials (ePCTs) of non-pharmacological interventions within health care systems to improve the care of people living with dementia (PLWD) and their care partners.

The IMPACT Collaboratory Pilot Grant Program funds pilot ePCTs that are designed to demonstrate the feasibility and inform the design of a future full-scale ePCT (Stage IV effectiveness based on the NIH Stage Model) to evaluate non-pharmacological interventions embedded in health care system(s) for PLWD and their care partners.

The IMPACT Collaboratory provides guidance for investigators in the design, conduct, and dissemination of ePCTs through its Cores.

Funding Opportunity Description

The IMPACT Collaboratory will fund up to three pilot ePCTs that are designed to demonstrate the feasibility and inform the design of a future full-scale ePCT for people living with dementia and their care partners. All applications must make a convincing case that the proposed pilot ePCT, if successful, can be scaled to a full-scale ePCT as the next research step. Required features of the study design are delineated below under General Requirements for Pilot ePCT Application.

Awards are for 18 months, up to $300,000 (total direct costs), and are non-renewable. Indirect costs are budgeted at your institution's negotiated facilities and administrative rate. Only a single Principal Investigator is allowed, and only one application per Principal Investigator is permitted in a single funding cycle.

NIH grants policies as described in the NIH Grants Policy Statement will apply to the applications submitted and awards made from this funding opportunity.

Team Activities

The Engaging Partners Team focuses on engaging community partners – including patients, care partners, and other interested parties - in all aspects of the development and conduct of embedded pragmatic clinical trials (ePCTs) among people living with dementia and their care partners.

Team members develop and disseminate guidance and training materials on engaging community partners in the conduct of ePCTs, and support IMPACT investigators on strategies for meaningful engagement of community partners throughout the research lifecycle. The Engaging Partners Team coordinates community partner input to setting-specific Health Care System Launchpads and engages PLWD and care partners as reviewers to evaluate proposals to IMPACT's pilot study program.

What is the Community Review Panel?

The Community Review Panel is a diverse community of people living with dementia or caring for people living with dementia who help make decisions about which research applications are recommended for funding by the IMPACT Collaboratory to the National Institute of Aging.

What does the Community Review Panel do?  

The Community Review Panel ensures that research to improve dementia care matters to people living with dementia and care partners. Members are asked to review proposals and answer the following types of questions:

  • Is this research important?

  • Does this research address an unmet need for people living with dementia and care partners?

  • Will this research make a difference in the real-world for people living with dementia, care partners, and health care systems?

Team Activities

The Health Equity Team (2019-2024) focused on developing and implementing strategies to address inclusion of people living with dementia (PLWD) and their care partners in the design and conduct of embedded pragmatic clinical trials (ePCTs). The team works to improve dementia care while ensuring the IMPACT Collaboratory is a national resource for all Americans affected by dementia.

Team members developed and disseminated guidance and training materials related to high-risk, high-need populations, provided guidance to investigators, and integrated with Core working groups to address the needs of high-risk, high-need populations in Core research activities.

Training Core

Training Core

Core Activities

The Training Core focuses on training investigators to become experts in conducting embedded pragmatic clinical trials (ePCTs) among people living with dementia (PLWD) and their care partners.

The Training Core funds and coordinates several training awards for investigators seeking to build careers in conducting ePCTs among PLWD and their care partners including:

  • Career Development Award (CDA) fund early-stage investigators for 2 years;
  • Health Care Systems Scholar Awards provide a year of support for researchers at any stage to develop a partnership with a healthcare system to design an ePCT.
  • Real World Data Scholar Awards are open to pre-doctoral and post-doctoral scholars to train in the use of databases important in ePCTs for PLWD and care partners.
  • Faculty Scholars join one or more of the IMPACT Collaboratory Cores over a year to learn about the Core’s focus and role in the development and conduct of ePCTs.

Core members develop and implement structured training programs that address all aspects of conducting these ePCTs. The programs include an annual Training Workshop and a collection of video training modules produced in collaboration with the other Cores and Teams. Core members also promote trainee expertise and productivity by integrating CDA recipients and other early-stage investigators into the IMPACT Collaboratory’s academic activities.

Training Workshop

Annual one and a half day program provides foundations in practical aspects of designing and conducting ePCTs in Alzheimer’s disease and related dementias (AD/ADRD) through a combination of activities including: panel discussions, small group sessions, and networking opportunities with experts in the field.

Core Activities

The Health Care Systems Core oversees and supports the four communities that make up the Learning Health Network—a consortium of individuals representing care settings and organizations joined by the common goal of improving care quality, equity, and accessibility for people living with dementia (PLWD) and their care partners.

The Core funds and coordinates a one-year Health Care System Scholars Program for researchers seeking to build a mutually beneficial partnership with a health care organization, with the aim of conducting ePCT research among PLWD and their care partners. Core members develop and disseminate best practices and setting-specific approaches to conducting these ePCTs. They manage the collaborative research resource involving leaders from diverse health care systems and provide guidance and training to investigators on partnering with these systems to facilitate successful implementation of interventions into clinical practice.

Implementation Core

Implementation Core

Core Activities

The Implementation Core focuses on implementing and disseminating dementia care interventions in the context of embedded pragmatic clinical trials (ePCTs) and optimizing the potential for integrating the intervention into health care systems.

A major focus of the Core is to collaborate with the other Scientific Cores and the broader scientific community in offering recommendations related to appropriate implementation models, strategies, and outcome measures/metrics when embedding dementia care interventions into various healthcare settings. The Core will also help to result in an overall synthesis of implementation success of Collaboratory-funded projects.

Patient & Caregiver Relevant Outcomes Core

Core Activities

The Patient and Caregiver Relevant Outcomes (PCRO) Core focuses on supporting the design and conduct of ePCTs by guiding investigators in use of outcomes that are both pragmatic and relevant to people living with dementia (PLWD) and their care partners.

Core members have expertise in established and emerging methods to measure outcomes using administrative data, clinical assessments, and data embedded in electronic health records.  The PCRO Core curates an online library of outcome measures relevant to conducting ePCTs. They provide guidance and consultation to investigators selecting outcomes for ADRD ePCTs, enhance investigator capacity through training and other resources, and describe and disseminate novel methods to address gaps in measurement and data capture for outcomes that matter to PLWD and their care partners.

Core Activities

The Pilot Studies Core focuses on soliciting, funding, and guiding pilot embedded pragmatic clinical trials (ePCTs) among people living with dementia (PLWD) and care partners within health care systems.

Core members provide mentorship, guidance, and resources to IMPACT investigators to ensure rigorous and timely conduct of funded pilot studies. They promote the expertise of these investigators by integrating them into the activities of the IMPACT Collaboratory, and assist investigators in transforming completed pilot studies into full-scale ePCTs.

    Core Activities

    The Design and Statistics Core focuses on biostatistical methods for the design, conduct, and analysis of health system-embedded pragmatic clinical trials (ePCTs) among people living with dementia (PLWD) and care partners.

    Core members develop and disseminate novel biostatistical approaches for the design and conduct of ePCTs; provide guidance to investigators on design, power and sample size, analytic methods and reproducibility.  They train the next generation of biostatisticians and collaborate with other Cores on the development of methods and tools supporting ePCT design.

    Ethics and Regulation Core

    Ethics and Regulation Core

    Core Activities

    The Ethics and Regulation Core aims to assure that embedded pragmatic clinical trials (ePCTs) in dementia care meet the ethical challenges and regulatory requirements for human subjects research with attention to the unique considerations of persons living with dementia (PLWD) and their care partners.

    Core members support the design and conduct of ePCTs among PLWD and their care partners by providing guidance and consultation to IMPACT investigators on relevant ethical and regulatory issues. Members also enhance investigator capacity by conducting training activities and mentoring faculty scholars and post-doctoral trainees on ethical and regulatory issues in conducting ePCTs among PLWD and their care partners. Additionally, the Core develops and disseminates knowledge to advance best practices and methodologies on topics related to ethical and regulatory issues pertinent to ePCT design and conduct.
